On certain computer systems, a software incompatibility between Internet Explorer (version 7 or higher), Google Toolbar, and Adobe Acrobat Reader can prevent our registration forms (and all other PDF documents that open in a new browser window) from loading. To work around this issue, you need to launch Internet Explorer with add-on components temporarily disabled:

Your web browser will launch with all add-on components (including the Google Toolbar) disabled. You will now be able to view/print our registration form.

Close your web browser after you are done viewing the registration form; all add-on components will be re-enabled the next time you launch it.
